Look-Back Review

After a problem, delay, mistake, or finished project.

The Look-Back Review is how a team learns from what just happened without turning the conversation into a search for who to blame. The goal is one small change that makes the same problem less likely next time.

When to use it

  • After a delay, missed deadline, or surprise.
  • After a finished project, large or small.
  • Whenever the same kind of problem keeps repeating.

How to use it

  1. Schedule 30 to 45 minutes. Invite only the people who were involved.
  2. Use the form below as the agenda. Walk through it as a group.
  3. End the meeting with one specific change, one named owner, and one date to check whether the change worked.

What good looks like

A good Look-Back ends with one written change, not seven. It does not name and shame. It does still hold people accountable when someone hid risk, lied, or refused to follow clear steps.
